Mail-in rebate: Connection card must be active for a minimum of 30 days to finalize processing of rebate submission. Rebate total may not exceed the total purchase price of the card. Connection Card purchases incident to automobile purchase are not eligible for this promotion. Reproduction, sale, trading or purchase of this certificate or proofs of purchase is prohibited. Use of multiple addresses or P.O. boxes to obtain additional rebates is considered fraudulent and may result in prosecution.
